Fatal Truck Mount Collapse in Glasgow

One man is dead and another seriously injured after the boom of a truck mounted lift buckled in Glasgow Scotland.

Details of what happened are sketchy, as it occurred less than two hours ago, but we understand that the two were working from a 43 metre Bizzochi lift on the refurbishment of Buchanan House in the city centre, when it occurred. The lift was on hire from a local company to One Stop Roofing, which was installing safety netting to scaffold.

Gareth Currie, 39, of One Stop Roofing, was pronounced dead at the scene, some reports have claimed that he was thrown from the platform, but none of them have been confirmed. The other man, the machine’s operator, Alex Nisbett, 35, did go down with the platform and was taken to hospital with severe head injuries – possibly caused by falling debris – according to the ambulance service.

The unit had its three section jib extended at the time and the third section of the boom buckled at the exit point from the second section.

Police, fire and ambulance personnel are still working at the scene.

UPDATE

We can now confirm that the aerial lift involveed was one previously owned by ES Access which was liquidated last year. Craig Services & Access acquired the machine and been running it since.

The unit was subjected to a serious overload a couple of years ago and was repaired in the factory just before ES Access went into administration.

The lift was eventually returned to the UK and sold by the administrator through a third party to Craig Services.

The lift then incurred more recent boom damage when the second section buckled. The unit was fully inspected under the manufacturer’s supervision. A repair was, we understand quoted for, but was eventually carried out elsewhere. We have not been able to ascertain who did that repair, or of course if it had any relevance to the current incident.

This is a tragic and almost certainly avoidable incident and warrants a full and in depth investigation that will truly get to the bottom of exactly what happened so that the industry can learn from it- and sooner rather than later.

Fatal Crane Incident in Chennai

8/14/2012 Update – Human Error to Blame

There has been another fatal crane incident on one of India’s major metro projects in Chennai last Wednesday night.

A crawler crane, a 75 tonne Tata TFC 280, was lifting 25 tonnes of reinforcing bar when the boom buckled, dropping the load from a height of around 22 metres onto seven construction workers waiting to receive it, killing one of them and injuring the other six.

The incident, which occurred at the Pachaiyappa’s College site, took the life of 30 year old Sindhu, while those injured were aged 20, 22, 25, 30 and 35. Their condition is said to be stable, although one is more critical than the others.

Police arrested the site manager for contractor Larsen & Toubro and the deputy project manager of Valecha Engineering, the contractor which supplied the crane as part of a sub-contract.

They were charged under four sections of the Indian Penal Code which comprised of section 287 – negligent conduct with machinery, 337 – causing hurt by act endangering life or personal safety of others, 338 – causing grievous hurt by act endangering life or personal safety and 304A – causing death by negligence.

The police also said that that they were also looking to arrest Amalesh Yadav, 22, the crane operator who has been missing since the incident occurred.

According to Chennai Metro Rail Limited several investigations have been set up including its own. There are also investigations from Larsen & Toubro and Chinese based Shanghai Urban Construction Group the main contractors for the station and another by the general consultant.

Work was also suspended on Thursday and a full safety audit of cranes on Chennai Metro sites ordered.

K. Rajaraman, managing director of Chennai Metro said: “Work has been suspended to put in tighter controls in place. We will have all the equipment re-certified. The audit is expected to take two to three days and construction work will resume soon after. This is the first time that work on the Metro line has come to a complete halt since the project was launched in 2009.”

Tower Crane Collapse in Thailand

Two construction workers were seriously injured and another knocked unconscious when a tower collapsed on a building site in Bangkok, Thailand on Friday.

The crane’s tower appears to have pulled out its anchors, throwing clouds of debris and some steel parts into the street and onto nearby houses. Having lost the support of the ties the tower gave way and folded in a twisting motion.

One man lost his right leg while a female employee had her right heel cut off. A third man was found unconscious on the site. All three were rushed to hospital for emergency treatment.

The crane was being used by TTS Engineering on a 26-storey building in the Din Daeng district. TTS manager Songwuth Suwannasirikul and the chief architect Sakchai Phalangsuriya were taken by the police for questioning.

Police said they are looking into the possibility of filing charges against the construction company.

11/2/2012 Update – What to do with the Tower Crane?

A construction crane atop a $1.5 billion luxury high-rise in midtown Manhattan collapsed in high winds Monday and dangled precariously, prompting plans for engineers and inspectors to climb to the top to examine it as a huge storm bore down on the city.

Some buildings, including 900 guests at the Parker Meridien hotel, were being evacuated as a precaution and the streets below were cleared, but there were no immediate reports of injuries. City officials didn’t have a number on how many people were told to leave.

Authorities received a call about the collapse at around 2 p.m. as conditions worsened from the approaching Hurricane Sandy. Meteorologists said winds atop the 74-story building could have been close to 95 mph at the time.

The nearly completed high-rise is known as One57 and is in one of the city’s most desirable neighborhoods, near Carnegie Hall, Columbus Circle and Central Park. It had been inspected, along with other city cranes, on Friday and was found to be ready for the weather.

Mayor Michael Bloomberg said later Monday it wasn’t clear why the accident happened.

“It’s conceivable that nobody did anything wrong and there was no malfunction, it was just a strange gust of wind,” Bloomberg said.

Engineers and inspectors were planning to hike up 74 flights of stairs to examine the crane. The harrowing inspection was being undertaken by experts who are “the best of the best,” city Buildings Department spokesman Tony Sclafani said.

Lend Lease, one of the largest construction companies in the city, is construction manager for the project. Bloomberg was careful not to blame the company, and said it would be days before officials figured out what happened.

A spokeswoman for Lend Lease said the company was working with city officials to secure the structure but the weather remained severe. There was no immediate response to a message left with the developer, Extell Development. A phone number for the company that owns the crane, Pinnacle Industries II LLC, rang unanswered.

The New York Times recently called the building a “global billionaires’ club” because the nine full-floor apartments near the top have all been sold to billionaires. Among them are two duplexes under contract for more than $90 million each.

Shannon Kaye, 96, lives in the building next door.

“We heard a noise, but we didn’t know what it was,” she said. Minutes later, she and her neighbors were told to leave.

“I never liked that building, looking down into my bedroom,” she said. “I always had the feeling that something would come falling down from it.”

The Buildings Department had suspended work at the building at 5 p.m. Saturday. It reminded contractors and property owners across the city to secure construction sites and buildings.

Bloomberg said the city inspected the site after the crane was secured.

“Just because it’s inspected doesn’t guarantee that God doesn’t do things or that metal doesn’t fail. There is no reason to suspect at this point in time that the inspection wasn’t adequate,” Bloomberg said.

City Department of Buildings records show a Sept. 21 complaint that a crane at the site was leaking oil onto the roof of an adjacent building; inspectors said a loose fitting was responsible. The fitting was being repaired and a cleanup was under way by the time inspectors arrived.

In April, the agency got a complaint that the heavy ball at the tip of a crane at the site came loose and hit the materials it was trying to lift, knocking some of them onto an adjacent building’s scaffolding. Officials stopped work at the site for a day and issued a violation notice, records show.

Construction cranes have been a source of safety worries in the city since two giant rigs collapsed within two months of each other in Manhattan in 2008, killing a total of nine people.

Those accidents spurred the resignation of the city’s buildings commissioner and fueled new safety measures, including hiring more inspectors and expanding training requirements and inspection checklists.

Another crane fell and killed a worker this April at a construction site for a new subway line. That rig was exempt from most city construction safety rules because it was working for a state-overseen agency that runs the subway system.

Like Monday’s accident, one of the 2008 crane collapses also centered on the rig’s long, mobile arm, known as a boom. In the May 2008 accident, the boom broke off a roughly 200-foot-tall rig, crashed into a nearby building and plummeted to the ground.

Prosecutors blamed that collapse on what they called a penny-pinching repair to a crucial component that lets the boom swivel. Lawyers for that crane’s owner, who ultimately was acquitted of manslaughter charges, said the operator made a mistake that tipped the boom over backward and snapped it.

An investigation is being conducted after a crane boom collapsed whilst lifting  concrete at City Gate, Valletta.

The Infrastructure Ministry said the accident happened around 10:45 this morning and the cause is not yet known. No injury or damage was caused as a consequence of the accident apart from the damage suffered by the crane boom.

The crane’s damaged boom is being dismantled because it is considered unsafe, given the weather forecast.

City Gate has been temporarily closed off until the boom is removed.

Pedestrians have been asked to use the access via the Central Bank annex and  Castille.

Access to City Gate will be reinstated by late evening.

It has attracted billionaire buyers and plenty of media buzz, but this kind of attention the developers of One57, New York’s soon-to-be tallest residential building, certainly don’t want.

All eyes are on the scaffolded high-rise as a crane hangs precipitously from its 90th floor. High winds from Superstorm Sandy caused the crane to collapse, spurring the city to shut down streets and evacuate nearby buildings.

On Wednesday, New York mayor Michael Bloomberg said the ties binding the crane to the building are secure, but added time will be needed to build another crane to remove the damaged one.

It’s a devastating setback for a building that has led a mostly charmed existence, said Jonathan Miller, CEO of New York-based appraisal firm, Miller Samuel.

Earlier this year, an unnamed buyer paid $90 million for the building’s more than 10,000 square-foot penthouse, the highest price ever paid for a New York City condo. The penthouse features 23-foot ceilings and a wall of windows overlooking Central Park, which is just two blocks away.

Prices for apartments in the 92-unit building start at $7 million. So far 60% of the units have been sold, according to Bovis Lend Lease.

The crane operator, Pinnacle Industries, did not answer its phones and developer Extell Development declined to comment and referred questions to the construction manager, Bovis Lend Lease.

Lend Lease said in a press release Thursday that it is working with structural engineers and city agencies to secure the crane.

The luffing jib tower crane that was damaged in last week’s Storms has now been secured.

The salvage team used a hand crank to slew the superstructure through 180 degrees so that the jib, which was pulled over the rear of the crane, faces the building.

They then used a series of cables to strap the jib to the building in order to ensure that it does not fall, allowing the area underneath to be reopened. The next stage is to install another crane or lifting gantry, in order to remove the damaged jib and help disassemble the crane.

Crane Snaps in Atlanta While Removing Macy’s Great Tree

A crane taking down the Macy’s Great Tree atop Lenox Square snapped in the process Wednesday morning.

WSB-TV’s Carol Sbarge went to the Buckhead mall, where the Christmas tree was partially hanging off the edge of the Macy’s roof around 9 a.m. Atlanta firefighters were at the scene.

Lenox Square security blocked off the north parking lot of the mall, and asked news media to leave the property.

There were no immediate reports of injuries.

Last year, the tree broke as crews worked to hoist it atop the department store. Officials said the tree was only a few feet off the ground when it cracked.

A 200-foot crane collapsed onto a building under construction near the East River waterfront Wednesday, injuring seven people, three of them seriously.

A two-story framework for the residential building in the New York City borough of Queens had been erected when the red crane toppled and went sprawling across it around 2:30 p.m. behind a big neon “Pepsi Cola” sign, a local landmark.

The three people who were seriously injured were in stable condition.

One person appeared to have a broken bone. Three people had to be extricated from underneath the crane, Deputy Fire Chief Mark Ferran said.

Preston White, 48, a carpenter from the Bronx, was on his first day on the job at the site in the Long Island City neighborhood.

He had turned to speak to a friend when he heard a popping sound and turned back around.

At that moment, “I saw the cable whipping toward the deck. … You could just hear it buckling,” White said.

The impact shook the scaffolding he was on.

The crane cut down the framework of the building “like a hot knife in butter,” White said, because there was no plaster on it yet.

A fellow worker, Russell Roberson, 32, of Brooklyn, said the crane had been up about four days — and went down really fast.

City officials went up in a cherry picker while investigating the accident.

Overloading blamed for NY crane collapse

The operator of the crawler crane that collapsed in New York on Wednesday has been blamed for overloading the crane.

A statement from the Department of Buildings claims that the crane was lifting a load of 23,900lbs (10.8 tonnes), when the crane’s maximum capacity was around half that amount.

As a result operator Paul Greer has had his licence suspended, while the investigation continues. The statement also says that the operator was unable to see the materials he was lifting and attempted to lift them outside the approved loading zone.

Seven people were injured in the incident, three seriously, work at the site remains suspended while the investigation continues.

Tower Crane Jib Buckles

The jib of a tower crane working on a construction site in Uetendorf near Thun, Switzerland buckled last week dropping its load into the site.

The crane, owned or contracted by the Thun based contractor Läderach Weibel, was lifting what looks like a skip full of supplies and materials at the site of a housing development when the jib of the Potain crane buckled. Fortunately no-one was injured in the incident.

The contractor while thankful that no one was hurt suggests that the incident was caused by the operator lifting an excessive load too quickly adding a dynamic loading to an already overloaded crane, thus causing the failure. However the hook looks to be fairly close in to the tower and it looks as though the trolley cable has caused the buckling.

No Outriggers Cause Fatality

The incident occurred at the premises of BMAC Wireless a manufacturer/installer of mobile phone towers. The man, 25, was apparently trained to used heavy equipment but decided that he was able to carry out the lift without setting the machine’s outriggers. This in spite of advice from a colleague.

Why is it that people repeatedly make this mistake? The boom truck looks almost brand new, it uses out and down A frame outriggers that take seconds to set and even if not required for the load provide additional security. This man’s short cut has resulted in his premature death and will have a massive effect on dozens of other people’s lives, from his immediate family to the other employees at the company and the firemen who had to clean up the mess.

Sadly the fire captain is quoted as saying: “It was tragic, a pure accident.” No I am sorry this was no accident it was a completely foreseeable, predictable event. It is certainly tragic for all those personally involved though.

Jib Buckles at Station

Eindhoven, Netherlands

The jib of a mobile self-erecting tower crane buckled at Eindhoven station in the Netherlands on Monday.

The six axle Spierings SK1265, owned by Van Riel crane rental, which has three locations in the area, was apparently lifting within its capacity at the time.

The 60 meter jib came down onto the railway causing the authorities to stop all rail traffic until the crane was made safe. Thankfully no one was hurt in the incident and it was due to be cleared in time for today’s commute.

Two Hurt in Crane Incident

Two men were seriously injured last week in the town of Bissen, Luxembourg when a mobile crane boom buckled.

We have received very little information, apart from a couple of photographs and the information that it occurred last Monday. We have managed to confirm that an incident did occur at that time, and that it was near the town hall of Bissen, which is located north of Luxembourg City.

Crane Fatality in Holland

A crane operator died this morning in Gouderak, near Gouda, the Netherlands, after the boom of a telescopic crawler crane broke away from the superstructure.

The boom twisted sideways and crashed down onto the operators cab crushing the operator.

Floating Crane Buckles

A floating crane working on the salvage of a sunken German mine layer from the second world war, collapsed yesterday while attempting to lift the final piece.

The crane, owned by De Klerk Waterbouw, was said to be making a 100 tonne lift when an unplanned movement caused the twin boom to crumple.

The boat, an MFP-920, is lying in around 14 meters of water near the shore of the Veerse Meer lake near Vrouwenpolder in Zeeland, southern Holland. It originally weighed around 240 tonnes, but had been cut into three sections, two of which had already been removed.

It has been lying upside in the lake since the war, but officials were worried that it might still have un-exploded mines on board, so it was being removed to go on show in a museum.

Boom Truck Collapse in Ohio

Lakewood, Ohio

The boom of a boom truck crashed down onto a garage in Lakewood, Ohio yesterday, there were no injuries.

The crane, owned by a tree service company Jim’s All Seasons, appears to have sheared or lost all of its slew ring bolts allowing the boom and turret to break free from the base. The boom came down onto two garages, but police said the damage was minor, although it did bring down some power lines. Thankfully there were no injuries.

The All Terrain crane owned by Emsley Crane Hire was apparently working on steel erection job at a new Tesco store when the incident occurred. While the cause could be down to a number of issues, it looks as though the boom sequence might have been inappropriate for the load being lifted at the time, with most of the heavier lower sections retracted or just partially extended, while the two lightest sections look as though they are fully out?

We understand that no one was hurt in the incident.

The following clarification and statement has been received from the company which explains what happened but also shows how the crane operator reacted calmly and professionally to the situation.Dear Sir

“Our crane was involved in an incident which you have featured and I’d like to clarify several points raised.

The 80 ton Demag crane was set on it its best working jib length duty for working between 18 and 20m radius. This is 20% of section 1, 20% of section 2 and 74% of sections 3 and 4, giving a jib length of 31.4m with the safe load code number 9.

This configuration was set to the method statement and lifting plan and has been confirmed and checked by site and the HSE. Unfortunately our crane was retracting piles with a pile extraction hammer under a CPA hire. Our crane was given a shock load from the hammer cuasing the jib failure. The crane driver realised there was a problem and reacted immediately by slewing round to a a safe area to rest the jib. Our company have immediately stopped this crane operation.”

Utah Boom Buckle

The boom of an All Terrain crane buckled while lifting a rooftop cooling unit onto an office building in Provo, Utah yesterday.

The boom buckled at the base

The crane, a 220 ton Grove GMK5275 (GMK 5220) owned by Arizona based Wagstaff Crane, was working with the second and third section retracted and all smaller sections extended. The buckling occurred at the base of section four. The crane, which has a seven section 68 meter main boom, was operating with the boom at a fairly low angle, possibly around 30 degrees.
If the rest of the boom was fully extended the crane would have been working at a radius of around 44 to 46 meters. The crane appears to have two counterweight sections installed – around 21 tons – if so the boom is limited to just under 50 meters and maximum radius 44 meters, depending on how much boom is out and capacity is in the region of 2.2 tons. Although the outriggers are short rigged so it may be lower.

The crane’s swing-away extension has dropped from its stowage brackets on the side of the boom, suggesting some sort of impact or impact. One report said that the crane tipped and then settled back onto its outriggers. If so it is possible that the buckling was caused by a dynamic shock load.
